The CNA works closely with the interdisciplinary hospice team to ensure that care plans are effectively implemented and adjusted according to patient needs. This role requires a deep commitment to empathetic care, excellent communication skills, and the ability to work independently in a home setting. Ultimately, the CNA contributes significantly to enhancing the quality of life for patients during their end-of-life journey while respecting their wishes and cultural values.

Minimum Qualifications:

  • Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin.

  • Offer compassionate emotional support and companionship to patients and their families, helping to alleviate anxiety and promote comfort.
  • Collaborate with the hospice care team to implement individualized care plans and participate in care coordination activities.
  • Maintain a safe, clean, and comfortable environment in the patient’s home, adhering to infection control and safety protocols.
  • Educate family members on basic care techniques and provide guidance on how to support their loved ones effectively.

Skills:

The required skills enable the CNA to deliver high-quality, compassionate care by effectively assisting with daily living activities and monitoring patient health. Strong communication skills are essential for interacting empathetically with patients and families, as well as for collaborating with the hospice care team to ensure coordinated care. Attention to detail and documentation skills are used daily to accurately record patient status and care provided, which supports clinical decision-making. Preferred skills such as familiarity with hospice care principles and electronic health records enhance the CNA’s ability to provide specialized support and maintain thorough patient records. Additionally, skills in time management and adaptability are crucial for managing visits across multiple home settings while responding to the unique needs of each patient.
